Moreover, prior to any treatment, a full case history is taken. This information is kept on paper only, along with your name, address and phone number.

All patient records and consent forms are kept in my office under lock and key.
I always destroy (by incineration) all records pertaining to any patient that has not attended clinic for a period of three years.
I never ask you for personal information unless I truly need it. (I can’t stand services that ask you for things like your gender or income level for no apparent reason).
I don’t share your personal information with anyone except to comply with the law, or protect my rights.
I don’t store personal information on my servers unless required for the on-going operation of my site.

It is my policy to respect your privacy regarding any information I may collect while operating my websites.
